Your challenge

• You will manage complex, multifaceted, and deadlocked cases and foster coordination among the parties involved in the fields of Health, criminal justice, and municipal affairs.
• You assess the situation, put the need for assistance and self-reliance into Map , and work with residents, their support network, and professionals to develop a support plan.
• You will coordinate specialized support, monitor progress and results, and make adjustments as needed.
• You act decisively in (impending) crisis situations, prevent escalation, and make difficult decisions when necessary.

Together we make it happen in Geldrop-Mierlo

Team Plus Social Domain, formerly known as the PlusTeam, will become part of the municipality of Geldrop-Mierlo effective January 1, 2027. Based in Geldrop-Mierlo, the team will continue to collaborate with the municipality of Waalre. We support residents of both municipalities with complex, multifaceted service needs. According to the current job posting, the team consists of approximately 30 professionals with diverse areas of expertise, including mental health care, care for people with mild intellectual disabilities, and addiction treatment. Together, we address complex service needs, with a focus on a systemic approach, collaboration, and shared responsibility.

What can you expect from us

• A salary of up to €5,714 gross per month (based on a 36-hour workweek), depending on your education and experience (salary level as of January 1, 2027). Your contract is governed by the Municipalities Collective Bargaining Agreement.
• Individual choice budget (IKB) of 17.05% on top of your salary.
• Variable working hours.
• Favorable pension fund and compensation for furnishing your home office.
• You will also have access to a phone and laptop, and electric bicycles are available for work appointments.
• An employment contract for the duration of one year with the prospect of permanent employment.

Who are you?

In addition to being a true go-getter who connects easily with others, you’re a good listener and have strong communication skills. You ask probing questions, look for solutions, and think in terms of possibilities. You have a relevant associate’s degree, an associate’s degree plus level of work and thinking, and at least four years of relevant work experience. You are SKJ- or BIG-registered. You possess analytical skills, social skills, problem-solving abilities, and the ability to handle stress. You can prepare detailed reports, organize complex situations, and collaborate with various parties. You are available for at least 28 hours per week and have a driver’s license and a car.
