Application from abroad
Do you live abroad and want to request an extract of civil status?
To request an extract of civil status from abroad, you will need the following documents:
- copy of ID.
- letter/statement provided with the year of birth, name and address and signature. The signature must match the signature on your ID.
Applications at front desk
If you are coming to request the document in person please bring the following;
- Valid proof of identity
- Debit card or cash. You pay immediately when you apply for the document. Preferably pay by debit card.
