What is it?

Municipalities record important events in a person's life in the civil registry. For example, a birth, marriage or death. You can request a copy or extract: a birth certificate, marriage certificate, divorce certificate or death certificate. This is written proof of these details.

How does it work?

Requesting a transcript or excerpt

You can request a transcript or excerpt from a:

  • birth certificate
  • death certificate
  • marriage certificate
  • divorce certificate
  • deed of partnership registration (no international deed is possible for this)
  • deed of termination of partnership (no international deed is possible for this)

Do you need a transcript or excerpt for abroad? Along with your application, you can also request a multilingual extract or multilingual model form.

What do I need?

When applying online, you will need:

  • your DigiD
  • internet banking data

Personal request

  • valid identity card
  • debit card or cash

Written request

  • A copy of your valid ID
  • A letter stating why you need the transcript or excerpt
  • your signature on the letter

Authorize someone else

The authorized representative takes your authorization letter to the municipality with:

  • A copy of your valid ID
  • his or her own proof of identity

How long does it take?

  • If you request the transcript or excerpt in person at the municipality, you will receive it immediately. This also applies if you authorize someone to do so.
  • If you request the transcript or excerpt online or in writing, the municipality will send it to you by mail.
