When might you get a socio-medical indication?

• You have a demonstrable physical, sensory, mental or psychological impairment;
• You live in the municipality of Geldrop-Mierlo;
• You have no or limited entitlement to child care subsidies from the tax authorities; and

Requirements

• Your child(ren) in the family are between the ages of 0 and 12.
• You are not entitled to child care money from the IRS or UWV.
• You are attending or receiving treatment or counseling.

What do you send along?

• copy of a valid ID (passport or ID, not a driver's license)

And if you have this:

• report from a social worker explaining your situation
• copy of signed child care contract with timesheet

Attachment(s) included

While filling out the form, we ask you to include several attachments.

It is helpful if you: have all attachments applicable to your situation available digitally. You can then upload these files directly - without having to search anymore. Consult the list of requested attachments to see which attachments apply to you.

Requested attachments:

• if already received: the quote from a child care organization
  The maximum size per attachment is 32 MB. Collectively the attachments may not exceed 128 MB. Allowed file types are: pdf, gif, jpg, png, jpeg, dsc, efdj, docx, xlsx.

You can also upload additional files to your application/case afterwards through the eloket.

What happens after your application?

Within 8 weeks of your application, you will receive a decision on reimbursement. Sometimes it may take longer if we still need data from you. We recommend that you wait to register your child with child care until you receive our decision.

Communicating change

Do you have an allowance (applied for)? And does anything change in your (financial) situation or in childcare? Please let us know. Also changes in your partner's situation.

For example, you communicate this to us:

• Something is changing in child care.
• You want to stop child care reimbursement.
• Your income changes.
• You are moving or living somewhere else temporarily.
• You want to change your bank account.
